This is a procedure of removal of spectacle number. While there is no upper limit for lasik, the lower limit is around 18 completed years of age and stabilisation of the spectacle number. LASIK can be used to treat a variety of spectacle numbers like minus, plus, cylindrical and presbyopic (separate reading and distance numbers).

LASIK is a procedure where the cornea (front transparent portion of the eye) is reshaped by a high precision laser to remove the spectacle number.

The process involves an initial evaluation of the patients number, retina a post dilated refraction and post dilated reconfirmation of spectacle number. Then a couple of tests are done i.e.TOPOGRAPHY (to evaluate the shape of the patients cornea) and PACHYMETRY (to scertain the thickness of the cornea). If all parameters are found suitable, LASIK is carried out.

The procedure is done in two separate ways:
This is one of the safest modalities of doing LASIK. Here the most superficial layer of the cornea(i.e. the epithelium) is first removed by laser. After that the cornea is reshaped by laser and the cornea is covered with a contact lens which may need to be worn for a few days till the epithelium regenerates.
Advantages :
  • It is a very safe procedure as flap related issues are absent in this form of LASIK.
Disadvantages :
  • It is associated with discomfort for three to five days till the corneal epithelium heals.
  • It is not advisable for higher numbers
This is the LASIK which is most commonly done because of the comfort and predictable outcomes. Here a superficial flap is made of the cornea and then LASER is applied to the internal cornea to reshape it. Then the flap is positioned back in place. There are two techniques of making the flap.

In BLADE LASIK, the flap is made by an automatic blade called a Microkeratome and in FEMTO (BLADELESS ) lasik, it is done by a separate laser machine. In simpler termes in the first there is a physical blade making the flap and in the latter there is a laser blade. Both techniques are good for a vast majority of patients. The femto lasik provides marginally better safety anf flap quality.
Advantages :
  • It is safe, Predictble and associated with very high comfort levels post Lasik.
  • It can be applied to variety of spectacle number.
Disadvantages :
  • It can be associated with Flap related issues and some dryness.
This is a recent advance where there is only a small laser cut on the cornea and the corneal reshaping is done by removal of a laser cut lenticule from this curved incision.
Advantages :
  • Safety is high.
  • Less Dryness.
Disadvantages :
  • High Cost.
  • Age completed 18 years
  • Stable number for at least six months
  • Off contact lenses for at least 14 days
  • Normal retina or after adequate treatment of any retinal disorders
  • No corneal abnormalities contraindicating LASIK
  • No severe dryness or autoimmune disorders affecting the eye
  • No eye makeup for a week
  • No contact lenses for at least two weeks
  • Hair bath on day of LASIK
  • No perfume , deodorant, cosmetics etc on the day of LASIK (smell interefers with LASIK)
Firstly the eyes are numbed by putting anaesthetic drops, then you have to lie down on the laser bed. All that is expected from you is that you have to Relax. Do not move the face or eyes, do not squeeze your eyes. Look at the fixating light in the laser machine.

Firstly the face is covered with a sterile drape and the eye is opened with a clip (speculum). This prevents your eyes from shutting. After that the eye is washed with saline and a flap is made. At this time there is a whizzing sound, the light disappears and some heaviness is felt in the eye. After this the light comes on again and then laser is started. This is the important part of the procedure where it is necessary for you to focus well on the light as better your focussing, faster and more accurate the process. Then the clip is removed and procedure is performed in the same fashion on the other eye.

For one day the vision is blurred and there is some irritation , discomfort and watering in the eye. It generally eases out after three to four hours. Vision usualy clears in a day. Devices can be used in a couple of days time, hair bath in a week, cooking after three days. Swimming and rubbing of eyes are not allowed for three months post lasik.
